Impotence and Diabetes
Impotence can be a problem for those with type 2 diabetes. The loss of sexual performance should always be discussed with your physician. It is also defined as "ED" or "Erectile Dysfunction".
"ED" in diabetes is caused by nerve damage in the penis. It is quite common and has been reported in up to 80% of men with diabetes. It is almost always organic in origin. Appropriate therapy for diabetes can never restore erectile dysfunction, because the diabetic process cannot be reversed.
In the past sexual impotence was regarded as a psychic problem, research has shown 85 to 90% of all cases have an organic cause such as peripheral vascular disease, diabetes, imbalance in sexual hormones and the effects of medication. Many drugs cause neurogenic impotence by affecting the neurotransmitters at the nerve endings. Some of these are anti-hypertensive and psycho tropics, although the list is very long.
Impotence is a common complication of diabetes. And left untreated it can put a lot of stress on a marriage or relationship. It can trigger depression, increase stress and even make it harder to manage your diabetes. It happens in 80% of men with diabetes and strikes at the heart of their manhood. A lot of men take part of their identity from their ability to perform sexually. This is especially important in marriage where intimacy is an important part of a relationship.
Another factor of impotence in diabetes is it affects men 10 to 15 years than the 65 years of age when it normally begins to appear. The longer you have diabetes and the more severe it is the more likely you are to develop "ED".
What Causes Erectile Dysfunction?
Impotence has many causes, both physical and psychological. Health conditions such as heart and liver disease can cause "ED" as well as many pharmaceutical drugs.
But when you have diabetes
-nerve damage
-blood vessel (vascular) damage
-poor blood pressure control
Are the main reasons for impotence in men.
The Erectile Process
Diabetes can cause neuropathy or damage to the nerves throughout your body. A look at normal male physiology show how these risk factors affect sexual ability.
The penis has two cylindrical sponge like structures that run down its length. These cylinders, the corpora cavernosum make up the bulk of the erectile tissue in the penis. The corpus spongiosum is a chamber that surrounds the urethra and becomes engorged with blood during an erection. An artery runs deep through the center of each corpus cavernosum, allowing blood to flow in. Blood flows back out through a system of veins around the outside of each corpus cavernosum.
Normal sexual function requires the interplay of both the nervous and vascular systems, as well as psychological events and physical and sensory events.
When you first become sexually stimulated the body releases neurotransmitters such as nitric oxide in the penile area. These chemical messengers tell the smooth muscle cells in the erectile tissue to relax. When they relax the central artery and other blood vessels widen and blood rushes into the penis. As the corpus cavernosa fill with blood the spongy tissue presses up against the veins, compressing them and preventing the blood from flowing out of the penis. This provides an erection as the trapped blood strengthens and stiffens the penis. When the stimulation ends, the muscles contract, pressure decreases, and the penis becomes flaccid, returning to its non erect size and shape.
How Diabetes affects normal function
In men with diabetes, a lot of things can go wrong, from the complications of diabetes mellitus, related to nerve and blood vessel damage. Damaged nerves can't communicate properly so information isn't relayed properly, and the penis doesn't respond.
In addition poor blood sugar control can affect and inhibit the production of nitrous oxide. This can prevent sufficient pressure on the veins so the blood drains out resulting in a poor erection. Also cardiovascular disease and narrowing of the blood vessels disrupts sexual function.
